Job Description

Job Summary

The Case Manager assesses client needs, develops individualized service plans, links clients to essential community resources, and coordinates medical, behavioral, and social services. This role is crucial for ensuring comprehensive care, advocating for clients, and promoting their stability and independence within the community.

Key Responsibilities

  • Conduct comprehensive needs assessments (psychosocial, financial, housing, vocational, and medical) to identify barriers to recovery and stability.
  • Develop, implement, and consistently review individualized Service Plans in collaboration with the client and the clinical treatment team.
  • Advocate for clients to access and retain essential community resources, including housing, food security, entitlements (e.g., Medicaid, SSI/SSDI), and educational/vocational training.
  • Coordinate all necessary appointments and facilitate transportation for medical, psychiatric, and therapeutic services.
  • Maintain detailed, accurate, and timely records of all case management activities, documenting progress toward service plan goals.
  • Provide crisis support and assist clients in developing independent living skills and effective coping strategies.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, or a closely related field.
  • Minimum of 2 years of experience in case management, resource coordination, or direct social service delivery, preferably within a behavioral health setting.
  • Demonstrated strong knowledge of local community resources, government entitlement programs, and health system navigation.
  • Valid driver’s license, reliable transportation, and current auto insurance for required client advocacy and travel.
