Reports To: Program Manager / Outreach Coordinator

Job Summary

The Community Health Worker (CHW) acts as a vital bridge between social services, healthcare providers, and the community, specifically for individuals facing barriers to care. The CHW provides advocacy, outreach, health education, social support, and referral/follow-up services to improve health literacy and facilitate access to necessary medical, behavioral, and social resources.

Key Responsibilities

  • Conduct proactive outreach and engagement within the community to identify individuals who are disconnected from health services and assist them in navigating the healthcare system.
  • Provide culturally competent health education and information on chronic disease management, preventive care, and mental health/substance use resources.
  • Offer one-on-one social support, motivational interviewing, and coaching to empower clients to achieve their personal health and wellness goals.
  • Facilitate referrals and linkage to essential services, including primary care, specialty care, mental health services, insurance enrollment, housing assistance, and food banks.
  • Assist clients in overcoming administrative barriers (e.g., completing paperwork, making appointments) and may provide limited, pre-approved accompaniment to appointments.
  • Document all client encounters, referrals, and outcomes accurately and confidentially.

Qualifications

  • High School Diploma or GED required. Associate’s or Bachelor’s degree in a health or social services field is a plus.
  • Certification as a Community Health Worker (CHW), if required by the state where services are delivered, is necessary or must be obtained within a set timeframe.
  • Demonstrated ability to relate effectively to, and advocate for, individuals from diverse backgrounds and underserved populations.
  • Strong knowledge of the local community resources and social service networks.
  • Excellent interpersonal, communication, and organizational skills; ability to work independently in community settings.
