Job Summary
The Housing and Homeless Services Case Manager provides comprehensive, intensive case management focused specifically on achieving and maintaining housing stability for individuals and families experiencing homelessness or housing insecurity, often those with co-occurring behavioral health challenges. This role involves assessment, housing placement, advocacy, and post-placement stabilization support.
Key Responsibilities
- Conduct thorough assessments of client housing needs, barriers to stability, and eligibility for various local, state, and federal housing programs (e.g., rapid re-housing, permanent supportive housing).
- Develop individualized Housing Stability Plans that integrate behavioral health goals with immediate and long-term housing solutions, financial planning, and independent living skills.
- Actively advocate for and assist clients in navigating complex housing application processes, securing necessary documentation, and liaising with landlords and housing authorities.
- Provide intensive, time-limited stabilization support immediately following housing placement to prevent recidivism into homelessness, including budgeting, tenancy skills training, and connection to furniture/utility assistance.
- Coordinate care with community providers, including mental health agencies, substance use treatment centers, and vocational training programs.
- Maintain accurate, timely, and compliant documentation, often including data entry into a Homeless Management Information System (HMIS).
Qualifications
- Bachelor’s degree in Social Work, Human Services, or a related behavioral health field.
- Minimum of 1 year of experience in homeless services, housing case management, or transitional living programs.
- Strong working knowledge of local housing resources, rental assistance programs, landlord-tenant laws, and the Housing First model.
- Proficiency with HMIS (Homeless Management Information System) or comparable client tracking databases is highly preferred.
- Excellent negotiation, advocacy, and crisis-intervention skills for working with high-needs populations.
