Job Summary
The Public Health Social Worker works at the intersection of individual clinical needs and broad community health issues. This role focuses on identifying, addressing, and mitigating the social determinants of health (SDOH) that impact the well-being of a target population. Key responsibilities include community needs assessment, program development, and advocacy for systemic change to improve health equity.
Key Responsibilities
- Conduct community needs assessments and data analysis to identify high-risk populations and prevalent social, economic, and environmental barriers to health (SDOH).
- Develop, implement, and evaluate health promotion and prevention programs at the community or systemic level (e.g., initiatives focused on reducing substance misuse rates or improving access to mental health services).
- Serve as a liaison and advocate between public health agencies, clinical healthcare systems, and community-based organizations to coordinate efforts and fill resource gaps.
- Provide culturally sensitive education and outreach to vulnerable populations on complex public health issues (e.g., infectious disease prevention, maternal health, chronic illness).
- Participate in policy advocacy, grant writing, and community organizing efforts aimed at addressing the root causes of health disparities.
- Provide limited, high-level case consultation and linkage to social services, utilizing a population-based perspective rather than individual clinical treatment.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited program. A specialization or concentration in Public Health or Policy is highly desirable.
- Current, active state licensure (LMSW or LCSW/LICSW, depending on the state’s requirements for public health roles).
- Minimum of 2 years of experience in public health, community organizing, health policy, or population health management.
- Expert knowledge of the Social Determinants of Health framework and experience utilizing public health data.
- Demonstrated ability in program evaluation, policy analysis, and cross-sector collaboration.
